New Patient Registration

Check if you are in our Catchment Area

Catchment Area

Reception will explain the practice policy and procedure for registration. When registering permanently you will be asked to complete a New Patient Information questionnaire so that we can plan your care more effectively and you are invited to make an appointment to see our Nurses for a routine screen.

On completion of our registration forms, you will be advised the name of your usual doctor; however, all patients are registered with the practice rather than an individual doctor.

Non-urgent advice:

Having a usual GP does not prevent you from seeing another GP in the practice, nor does it mean that you will always see your usual GP, as GPs are not available every day the practice is open.
