New Patient Registration
If you wish to apply to join the practice, please ask at reception and bring with you photo ID and recent proof of current address. Reception will explain the practice policy and procedure for registration. When registering permanently you will be asked to complete a New Patient Information questionnaire so that we can plan your care more effectively and you are invited to make an appointment to see our Nurses for a routine screen.
On completion of our registration forms, you will be advised the name of your usual doctor; however, all patients are registered with the practice rather than an individual doctor.
Having a usual GP does not prevent you from seeing another GP in the practice, nor does it mean that you will always see your usual GP, as GPs are not available every day the practice is open.
NUFFIELD HOUSE SURGERY POLICY – We do not accept patient requests for change of their usual GP, as patients are able to choose to see another GP for pre-bookable appointments.
Patient Registration Area
Nuffield House is situated in The Stow and serves the area of Mark Hall North, Mark Hall South, Netteswell, Potter Street, Church Langley, Newhall and Fifth Avenue. Patients living within these areas can apply to be registered at the practice. Patients already registered with the practice that move to an address South of Southern Way, and North or West of Elizabeth Way and North of Edinburgh Way must register with a practice nearer their new home. Click here to view the Practice catchment area (solid red line indicates boundary for new registrations). If in doubt about our catchment area, please ask at Reception.
Guide to GP Services
The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them. You can download the guide below.
