• Be entitled to NHS care in the UK
• Be living within our catchments area.
• You must provide proof of your address dated within 4 weeks e.g.  a current utility bill, rent book or bank statement.
• It is helpful if you bring your NHS medical card.
• If you are from abroad you may be asked to show your passport or other relevant paperwork.

Once we have confirmed the above and have the relevant documentation above you will then be required to complete Registration forms.

You may be asked to come back another time if the surgery is busy and there is no one available to deal with your application.

An appointment cannot be made until the above criteria has been fulfilled and your details have been entered onto the computer. This may take a couple of days.

When you are registered with The Practice you will be invited  to make an appointment for a new patient health check.
